How small can you get a QR code and how do you go about getting one?

I am revealing a rough draft of my idea for a TN info card. I have consulted with some members and incorporated some ideas from this site and this discussion.
The drawing on the front by, Edvard Munch, reflects the lightning strikes and burning pain that make us scream. The position of the hands are the way I hold my hands when the pain attacks. I know others have held their hands in the same way, hoping to help decrease the pain.
What we need is maybe three websites to put on the card.
It has been suggested by a member that it should be” intense boring burning pain” instead of “ dull burning pain”, I like that also.
I know nothing about the QR symbol so I could use help with that.
It is impossible to explain to people what we are going through. My co workers just cannot understand why I sometimes have to stop talking mid way through a sentence. Why at times all I can do is hide in the bathroom crying my eyes out, which of course only makes the pain worse. I call it my nerve pain or my face thing to those who actually do care about me and want to know when I’m having an episode. If you come up with a card I would be happy to carry one…
Hopefully this draft will soon become a card that you can carry.

We've developed an Attending Physician Advisory form on one type-written page. If you can figure out how to get the essential data on the back of a wallet sized card, then you're a better writer than I am, Scott. Feel free to take a look at the form. It's under our Face Pain Info tab.
Maybe that can be put under another QR symbol that can be accessed by a smart phone. We can possibly put two of the QR symbols on the card. Any thoughts on this by anyone would be helpful.

FYI, the US TN Association sells a smart-card based electronic Medical Information card that plugs into a computer USB port. You can look it up in their bookstore, if I recall correctly. (http://www.fpa-support.org). The data set on the card amounts to eight pages -- too long for a lot of patients to fill out when they're medicated. There needs to be a happy medium concerning data volumes in this sort of thing, I think. But I would certainly welcome another set of eyes and thoughts on this area, Scott. So have at it.
This is a great idea! It takes too long for me to describe my problem to people and they start to not care or stop believing I have it. They just don't get that it can be constant pain. When I describe the burning I have they always ask "does it burn right now?"....well I just told them it burns all the time. And they don't understand the medication that goes with it and how that makes me feel awful and I still have the pain. Love the idea!
Hi Scott I think this is a fabulous idea and will be so helpful to so many people. I work as a web designer so I know a little about QR codes feel free to message me if you need any help at all.
The main points are that the QR code has to be readable on a business card. However they vary in sizes depending on how many characters you are embedding. The best way is to embed a URL but put it through a tiny link generator first to reduce your characters.
I was trying to think of a way everyone could have their details within a URL without them having to have their own site or knowledge of websites. One way I thought was perhaps a section could be created within our profiles here to have all the important info in a concise and easy to read layout. Obviously don't really know too much about the way this site is built but just thought I would throw it out there.
Pixip: Do you know of any way to guarantee that the kind of patient record you are describing cannot be hacked under the NING operating environment?
