I haven't seen any papers dealing with failure analysis in MVD. The three factors I have heard from patient reports most often are pads that "slipped" for reasons unknown, excessive handling of the nerve that resulted in additional damage, and failure to explore all the way around the nerve and identify all impinging blood vessels. The latter issue comes up in papers that evaluate procedure results using the "endoscope" versus "microscope".

On some websites, I have read that some inexperienced surgeons find one blood vessel compressing the nerve but don’t catch others that may also be compressing the nerve. Sounds simple, but it is not easy to see all around the nerve. Getting another MVD by an experienced surgeon is often prescribed to fix this. Also, I have read that having the MVD many years after pain starts can lower success rates, perhaps because nerve is damaged as well as compressed.
