Any thoughts on an upcoming Dental extract?

Here is the situation and not sugar coated .

I have been Dxed with TN on my left side - not shooting electrical - deep crushing episodes triggered mostly by weather changes and over use of facial muscles . Not constant - usually lasting 30-45 mins sometimes every 2 hours all day and night.

This all started about 3 years ago with a tooth ache on a broken tooth (upper left). It was removed - rather roughly if I might say. I had a car accident when I was younger and broke my lower Jaw bilaterally and pretty well screwed up my whole mouth .

A lifetime of smoking and drinking has left me with worsening periodontal problems .

So I had been having some pain on my right side with a particular tooth and it is definitely a 'bad tooth'

Went to the dentist yesterday , a new one since the other guy retired. He looked it over did some X-rays and it has absessed , I could see the absess . I am 53 by the way . So the Dr. says the tooth has to go ,there is no saving it . Wants to pull it . The problem is a cavity on the side of the tooth .

He mentions that bone is receding and the roots go into my sinus .

This just scares me because this is similar to how my problems seemed to start on the other side.

I am worried about the local anesthesia (needles) into my nerves again , and possibly other things beginning again on this side .

This doesn't feel like it did before but I worry that this is going to go bad .

I did talk about my recent TN DX and MRI coming up soon , but It was kinda like getting a blank stare as if those were not considerations.

I don't know if he even understands my anxieties about this , I am not so worried about triggering pain on the left , but more about starting another vicious circle.

I have to get the right side fixed ... soon .

My instincts just tell me something is wrong here.

I would appreciate any thought you might have or other options I should think about.

I am taking an antibiotic for a week and other meds to help with the pain , Scheduled for Next Tues.

BTW paid in advance , which I found rather strange as well.

I just wanted to say I have had 4 teeth pulled since my diagnosis of A.O. I've had two pulled on my upper left side and two on my upper right side...The right side I had no increase in my pain. When I had the top left side done it caused my left side gums to sting for about three days..That was the only extra pain I had..Plus less anxiety because my bad teeth are gone..I have had teeth in my sinuses also. Most dentist can repair sinus perforations right after the extraction. And alot of them heal on their own with no other help..You just can't blow your nose or sneeze for awhile...It's all so scary , I know..I need some more dental work and am not looking forward to it.....Did you read the ''face pain info"? at the top of the page? It gives tips on what to do before a dental appoinment.....

Is your dentist a oral surgeon? I would go to an oral surgeon and have him give you anesthesia, and let him know about your problem. I think it would make you feel better if you were given anesthesia. Just make sure you discuss your TN with whom ever you choose. Good luck.

Rushing out to dr. appt so I will be brief but I would only allow an oral surgeon who is familiar with TN to do an extraction. Get the best oral surgeon in town. I have had several extractions and you do not want a regular dentist doing it, especially one who gives you a blank look when you talk about TN. Cleo might chime in here, I hope she does.
