Sphenopalatine Ganglion Block

Hello Everyone,

I have TN2 pain all day every day for the last 18 months due to a root canal procedure. I have tried 5 of the typical meds, seen 8 doctors, acupuncture, upper cervical chiropracter, etc.

I had a Sphenopalatine Ganglion Block this morning and I am shocked. I have gone almost 2 hours with no pain.

If you have not tried it, please consider it. My doctor does the approach using a catheter through the nose instead of needles.

Hello Brad, I just wanted to say that I also have great luck with the Sphenopalentine ganglion block. Few months back I had it done transnasal and it was so successful that my pain doc actually sent me home with a prescription of the medication that I can administer myself as needed. A few years ago, they were still giving these through the cheek with a cannula. I had one that was not so successful and only lasted about 4 days. the pain from the injection lasted a couple of weeks so that one was no bueno. But Transnasal is the way to go. Bottle says 4% Lidocaine and 1%phenylephrine. Great stuff for tn2.

Is your pain upper teeth? I have had a “tooth ache” upper left teeth and neck pain, left side, for over 20 years. Had MVD surgery 1.5 years ago. Didn’t work. Wondering if this might work. Thank you!

Wow! How long did your first one last? It has been about 4 hours and I feel like the pressure is coming back... Do they last longer each time?

Yes my pain is upper teeth! Please give this a try! If I understand right, each time you have it administered it can last longer.

Hey Brad! How are you feeling today? I have been contemplating having the Sphenocath done, but the closest doctor who performs them is 4 hours away from me. Did you have yours done under fluoroscope? I have constant pain in my temple from type 2 and also shocks in V2 and V3. I’ve had 2 MVD’s, a glycerol rhizotomy, and a nerve resection of a sensory nerve in my temple that only made me numb in a tiny section of my temple, so I’m still in constant pain. I’d love to hear how you’re feeling a day later. Thanks!

They last anywhere from 1 day to 2 weeks. All depends on flare up. I have determined that my sinuses are a large part of my TN1/TN2 pain. I have learned diet contols my sinuses and a lot of nerve pain. So by keeping sinus pressure at lowest point I win.

No fluoroscope needed for transnasal procedure. I actually preform them on myself now.

Yes it was with fluoroscopic guidance. It lasted 4 hours for me, an amazing pain free 4 hours. The hope is that each time it lasts a little longer. I go in tomorrow to have it done again. I am hoping and praying it works again and holds for a longer amount of time.

Update:

I could not get the block to last more than 4-6 hours at a time. But now my neurosurgeon/neurologist has more information on what may be effective in the future.

Brad has your neurologist given you the information as to what can be done for you in the future,if so can you share it please?

I am now on Cymbalta, Carbamezapine and Lyrica. The pain is more manageable but I still have it everyday.

I'm very happy to see you got relief! I did not. I had 4. The first was to soon to tell, the second lasted about a week and the 3rd about 3 days. The same for number 4. Of course we were looking for longevity.

I had a nerve block done yesterday. By last night the pain was so bad that I had to take an opioid. Today the pain is back to the usual daily upper teeth pain and burning. So I guess it didn’t work. :frowning:

so sorry to hear about your discomfort. I didn't get the intended results from that procedure.

How long did the relief last? It lasted 4 hours for me. It is more of a diagnostic tool I am finding.
