Most neurosurgeons can do a glycerol rhizotomy, TMB. The procedure tends to help most patients for about a year before the pain comes back. RF Rhizotomy tends to have a significantly longer-lasting effect than glycerol, but most physicians will resist doing multiple procedures if pain has recurred.
You might want to contact the University of California Medical Center in San Francisco. A former member of the Medical Advisory Board for the US TN Association has been in practice there and has trained a fair number of younger doctors in all of the procedures used for chronic facial pain.
